WELCOME

Welcome to Child & Family Guidance Center. We would love to help you begin your mental health journey to wellness. We know that often times taking the first step is the hardest step. We hope to break down the process and decrease any anxiety about what to expect at Child & Family.

Whether you, as an adult, are seeking services or you are visiting for your children, our goal is to make the intake process as seamless as possible. Prior to calling us for an appointment, here is a list of items to gather:


  1. Insurance Information - have your health insurance card accessible and be ready to provide information. 
  2. No health insurance? No problem! At Child & Family Guidance Center, no one will be turned away for their inability to pay. We accept Medicaid, CHIP, Medicare, and most private insurance plans (see list here). We work with individuals at all levels of financial need to identify the best option to cover cost of services.
  3. Once insurance information is obtained, contact us to schedule an initial intake appointment at your location of choice. In most cases, this appointment will last approximately one hour and is scheduled with a Licensed Professional Counselor. Together, you and the counselor will determine a treatment plan of action and, if necessary, schedule a separate appointment with a Psychiatrist.
  4. ​After your intake & initial Psychiatric Evaluation, your ongoing treatment will begin. Based on your plan and treatment needs, services will begin either in home or in office. It is important to maintain services to optimize your mental health goals. 

Below are the necessary forms to be completed prior to your first appointment. If you're able, download, print and bring them with you. This will help save time by reducing wait time.
