YOUTH EMPOWERMENT SERVICES
Services & Goals
Youth Empowerment Services are community-based services to children and adolescents with severe emotional disturbances. Children at risk for out of home placement are served by a team of mental health professionals who create an individual treatment plan and utilize a wraparound treatment approach to encourage and sustain family placement. By utilizing the wrap around process and accessing area resources this service model allows CFGC to maintain our program goals to:
Reduce out-of-home placements and decrease inpatient hospitalizations.
Provide a more complete continuum of community-based services and supports.
Ensure families have access to parent partners & support services.
Prevent entry into the foster care system or relinquishment of parental custody.
Who is Eligible?
Services are provided in the following counties: Collin, Dallas, Ellis, Hunt, Kaufman, Navarro, and Rockwall. Families seeking services should meet the following criteria:
Children ages 3-18 years old.
Reside in a non-institutional setting.
Serious functional impairments that include:
Risk of Harm
Poor School Behavior
Current Mental Health Diagnosis
Expectation that the child or adolescent would qualify for inpatient care.
Child or adolescent must be eligible for Medicaid.
